Patient Cost Sharing
Co-Payment - Some insurance companies require Heritage Valley to collect co-payments from patients for specific types of medical services. Co-payments are most commonly associated with Physician Office visits, emergency visits, behavioral health visits and outpatient rehabilitation visits. Co-payments that are mandated by your insurance company are payable at the time of service. Co-payment amounts not only vary by insurance company, but they also vary by group or benefit plan. Your Co-payment amounts are part of the benefit contract between you (the patient or subscriber) and your insurance carrier.
Coinsurance – Your
share of the costs of a health care service.
It’s usually figured as a percentage of the allowed amount. You start paying coinsurance after you’ve
paid your plan’s deductible.
Deductible - A set dollar amount you pay for health care services before your health insurance begins to pay.
All patients should be aware of their financial obligations prior to seeking services and should contact the insurance carrier or their employer with any specific questions.
